Freezing Your Elevations Membership

Your Elevations membership may be frozen for up to six months.  A freeze fee of $10/month applies.  If you are still within your initial membership term, the months you are on freeze will not apply toward your term. For medical freezes, please call 888-827-9262. 

The form, below, may be used to request a freeze.  Please submit one form per person. Freeze requests must be received at least 14 days before your next billing, otherwise your freeze will begin on your next available billing date. A re-activation date must be provided.  Please note your re-activation date as Elevations does not send reminders when regular billing begins.

We will respond to your request within a few business days to confirm your request or address any problems.

Note: If you are requesting to freeze due to medical reasons, a doctor's note will be required, and must be provided in order for us to waive your freeze fee. After we receive your freeze request, you will receive confirmation and instructions (by email) on how to submit your medical excuse.
