Cancelling Your Elevations Membership

The form below may be used to submit your Elevations Health Club cancellation.  Please submit one form per person.

Cancellation requests must be submitted at least 14 days prior to your next billing date, otherwise your cancellation will be executed in the following month. Elevations or our Member Services group will contact you within a few days of receiving this form in order to confirm your cancellation or address any problems that may need to be resolved.

Please note that if you are in your initial 12-month contract term then specific circumstances, in accordance with your Elevations contract, must exist in order to cancel. Specifically, you must be relocating more than 25 miles from the club, or you must have a medical issue that would prevent you from utilizing 1/3 or more of the facility for at least 6 months or the remainder of your term. This does not apply after the initial 12-month contract period.

Elevations Membership Cancellation Request

Cancellation requests must be received at least 14 days prior to your next billing date in order to prevent that billing. Otherwise, you will be canceled as of the next available date. *

Your account must be in good financial standing, with no outstanding balance, in order to cancel. *

Members who wish to cancel during their initial 12-month contract period may do so only for a) medical reasons or b) relocation. Specific circumstances, in accordance with the Elevations contract, must exist, and a doctor's note or proof of move will be required in order to complete the cancellation process. This does not apply if you are not within a 12-month contract period. *
