Cancelling Your Elevations Membership

The form below may be used to submit your Elevations Health Club cancellation.  Please submit one form per person.

Cancellation requests must be submitted at least 14 days prior to your next billing date, otherwise your cancellation will be executed in the following month. Elevations or our Member Services group will contact you within a few days of receiving this form in order to confirm your cancellation or address any problems that may need to be resolved.

Cancellation requests must be received at least 14 days prior to your next billing date in order to prevent that billing. Otherwise, you will be canceled as of the next available date. *

 
Your account must be in good standing, with no outstanding balance, in order to cancel. *

 
Note: Members who wish to cancel during their initial 12-month contract period may only do so due to relocation or for medical reasons. Proof-of-move or a doctor's note will be required, and must be provided in order to complete the cancellation process. After we receive your cancellation request, you will receive confirmation and instructions (by email) on how to submit proof of move or a medical excuse.
